For domestic EMS items:

From:
Sender's address should be filed in English or in Russian (Postal code, address of the company o sender, last name, first name, number of the telephone). Sender should sign the label.
To:
Addressee's address should be filled in English or Russian (Last name, first name, name of the company, address and phone number).
Name of the country should be repeated in Russian. In addressee's address postal code and telephone number should be indicated.
Customs declaration:

Insurance amount
Should be filled by operator.

Date
Should be filled by operator. In proper sections date of reception of EMS item is indicated. The operator who has excepted EMS item, should put the signature and a print of a calendar stamp or a stamp of the enterprise of reception of item.

Fee
Should be filled by operator in accordance with the rates of EMS Uzbekistan. If the item is insured, the sum of gathering for the insurance is underlined. In the column of All the total sum in view of cost of departure and the insurance is underlined.

The address details should be written in the following sequence
- addressee's details for citizens-Last name, first name and for legal persons-the name of legal person
- house number, street name, apartment number
- postal code
- place of residence (city, village etc)
- region name
- country name
- the sender's and recipient's phone numbers
